HWY 99; STOCKTON 95205 <br /> The San Joaquin County Public Health Services, Environmental Health Division <br /> has recently been included in the State Water Resources Control Board (SWRCB) <br /> UST Pilot Program for oversight of assessment and clean up activities at <br /> contaminated underground tank sites. <br /> A proposal for continued assessment and/or remediation for the above <br /> referenced site has been submitted to this office. <br /> In order to expedite the continued field investigative activities and to issue <br /> local permits, the following information should be submitted as an addendum to <br /> the proposal: <br /> 1. A proposed program, including methodology, to define the extent of <br /> soil and groundwater contamination. <br /> 2. A proposal to dispose of soil boring(s) cuttings and/or bailed water <br /> volumes from monitoring well(s) . <br /> All proposals and reports submitted must be accompanied by a Letter of <br /> Acknowledgement from the responsible party(ies) which states, at a minimum, <br /> the following and incorporates the enclosed Regional Board comments: <br /> I(We) declare, under penalty of perjury, that the information <br /> and/or recommendations contained in the attached proposal or <br /> report is/are true and correct, and that all work and reports <br /> which required geologic or engineering evaluations and/or <br /> judgments have been performed under the direction of an <br /> appropriately registered or certified professional. <br /> The attached proposal or report utilizes the "Regional Board Staff <br /> Recommendations For Initial Evaluation and Investigation of <br /> Underground Tanks" and "Appendix All reporting format.
