This facility's EPA ID n mber is inactive and has the incorrect owner/operator associated with the <br /> number. A hazardous waste generator shall not treat, store, dispose of, transport or offer for <br /> transportation, hazardous w ste without an EPA ID number. Immediately contact California Dept of <br /> Toxic Substances Control (D SC) and reactivate your EPA ID number, update the owner/operator <br /> information and submit evidece to the San Joaquin County Environmental Health Department (EHD) by <br /> April 15, 2012. DTSC may r quire a new EPA number for a new owner/operator. A form has been <br /> provided. <br /> 703. An emergency coordina or and modified contingency plan information is lacking. There must be at <br /> least one emergency coordin for on site or on call to coordinate emergency response measures, and the <br /> following information must b posted by a phone: the name and phone number of the emergency <br /> coordinator; location of fire e tinguishers, spill control equipment, and if present, fire alarm; and the <br /> phone number of the fire department, unless the facility has a direct alarm.
