Priority Class D is the lowest priority and <br /> will not be funded for quite some time. Priority Class D claims have a $10,000 deductible while <br /> Priority Class C claims have a $5,000 deductible. I believe you would qualify as a Priority Class <br /> C as your previous claim for the contamination associated with the 4,000 gallon gasoline tank <br /> removed in 1995 (Claim No. 010934) is a Priority Class C claim and is currently being funded. <br /> To request Priority Class C, correct pages 5 and 6 of the claim application and provide <br /> documentation of the number of full-time and part-time employees of T & T Trucking, Inc. (e.g,, <br /> DE3 form submitted to Employment Development Department quarterly). <br /> You are required to demonstrate financial responsibility. The copy of the Certification of <br /> Financial Responsibility that you provided is dated November 8, 1993. You did not include the <br /> Chief Financial Officer Letter. This is not acceptable. Enclosed are forms and instructions. <br /> Questions regarding financial responsibility should be directed to Judi Nash at (916) 227-4527. <br /> You have indicated that the 10,000 gallon diesel UST was removed after the December.22, 1998 <br /> deadline to have all single walled USTs removed or upgraded. You must demonstrate that the <br /> following actions were completed prior to December 22, 1998: <br /> 1. Operation of the UST had ceased. <br /> 2. UST was emptied. <br /> 3.
