(AGE) on your behalf for <br /> the above-referenced site. By letter dated 03 August 2012, the EHD had directed the submittal <br /> of a scope of work proposal that would describe the methods and materials to be used to <br /> destroy the wells at your site. <br /> In the work plan addendum, AGE proposes to destroy monitoring well MW-1, soil vapor <br /> extraction well VEW-1, and air sparge well ASW-3 by drilling out the well contents to one foot <br /> deeper than the total depth of each well using either 8.25-inch or 10-inch diameter hollow-stem <br /> augers. AGE proposes to destroy the remaining wells (MW-2 through MW-4, VEW-2, VEW-3, <br /> ASW-1, ASW-2, ASW-4 and ASW-5) by pressure grouting. <br /> The EHD approves the work plan as submitted. Please submit a Well and Boring Permit <br /> Application form, Master File Record form and permit/inspection activity fees of $1,875.00 to the <br /> EHD. Notify the EHD at least a week prior to the scheduled date for destroying the wells.
