l F ` <br /> MEDICAL SURVEILLANCE REQUIREMENTS <br /> Medical surveillance is conducted as a routine program for field staff which meets the <br /> requirements of 29 CFR 1910.120 (f). This program includes baseline medical examinations for <br /> field work certification and annual follow-up examinations. Special medical tests or examinations <br /> are not anticipated for field personnel assigned to this project. The medical surveillance program is <br /> conducted under the guidance of an occupational medicine physician who established the medical <br /> exam protocol and certification requirements. <br /> All subcontractor personnel directly involved with the field work must also meet the <br /> medical surveillance requirements of 29 CFR 1910.120. <br /> f SITE CONTROL MEASURES <br /> This section describes the general facilities and site-specific control measures for this <br /> project. The potential chemical and physical hazards have been identified in this SSHP; however, . <br /> should unexpected conditions arise, the SSO will stop all work at the site and notify the PM and <br /> HSD. Work will not resume until the SSHP and working conditions have been reevaluated and the <br /> SSHP revised accordingly. <br /> A map of the site is included as Figure 1, the exclusion area(s), decontamination area(s), <br /> and support area(s)for the various field activities will be designated at the time of drilling. <br /> Communication between field team members will consist ons st of verbal communication and <br /> hand signals (if necessary): <br /> The nearest medical assistance.is Lodi Memorial, located.at 975 South Fairmont Avenue, <br /> Lodi,telephone number: (209) 334-3411. <br /> Work Practices <br /> k <br /> Safe work practices are part of assuring a safe and healthful working environment. Set up, assemble, and check out all equipment for integrity and proper function prior to <br /> starting work activities. <br /> 2.
