Jerry Moore
Page 2
Pursuant to Section 2652(d) of the Underground Storage Tank Regulations (California Administrative Code,
Title 23, Subchapter 16) a quarterly monitoring report shall be submitted to Public Health Services,
Environmental Health Division and Central Valley Regional Water Quality Control Board (CVkWQCB) until
the investigation and cleanup of the site is deemed adequate. The report is to show the:
-status of investigation and cleanup activities including the results of all investigations
completed to date;
-proposed continuing or next phase of investigation;
-method of cleanup proposed or implemented, schedules for the completion of the investigation
of the site and remediation;
-monitoring well data including groundwater elevation, pH, conductivity, gradient;
-analytical results of groundwater or soil sampling; and
-a site map showing the "zero Iine"of contamination, if known, and changes in the above
measurements over the Iast quarter.
When investigation schedules and report preparation overlap, the quarterly monitoring data may be
submitted as a letter to the regulating agencies.
After the lateral and vertical extent of soil and groundwater contamination have been physically confirmed,
an addendum to the PAR is to be submitted and a final remedial/corrective action plan is to be prepared.
If you have any questions, contact Margaret Lagorio, Senior RENS, at (209)468-3450.
Jogi Khanna, M.
