Jerry Moore <br /> Page 2 <br /> Pursuant to Section 2652(d) of the Underground Storage Tank Regulations (California Administrative Code, <br /> Title 23, Subchapter 16) a quarterly monitoring report shall be submitted to Public Health Services, <br /> Environmental Health Division and Central Valley Regional Water Quality Control Board (CVkWQCB) until <br /> the investigation and cleanup of the site is deemed adequate. The report is to show the: <br /> -status of investigation and cleanup activities including the results of all investigations <br /> completed to date; <br /> -proposed continuing or next phase of investigation; <br /> -method of cleanup proposed or implemented, schedules for the completion of the investigation <br /> of the site and remediation; <br /> -monitoring well data including groundwater elevation, pH, conductivity, gradient; <br /> -analytical results of groundwater or soil sampling; and <br /> -a site map showing the "zero Iine"of contamination, if known, and changes in the above <br /> measurements over the Iast quarter. <br /> When investigation schedules and report preparation overlap, the quarterly monitoring data may be <br /> submitted as a letter to the regulating agencies. <br /> After the lateral and vertical extent of soil and groundwater contamination have been physically confirmed, <br /> an addendum to the PAR is to be submitted and a final remedial/corrective action plan is to be prepared. <br /> If you have any questions, contact Margaret Lagorio, Senior RENS, at (209)468-3450. <br /> Jogi Khanna, M.
