Enclosed <br /> please find. a signed Pre-Approval Request Form; a detailed cost estimate; a signed Authorized <br /> Representative Designation form; and San Joaquin County Environmental Health Department <br /> (SJCEHD) letter with regards to the off-site domestic well. <br /> The purpose of this'pre-approval is for the replacement of the off-site domestic well at 14852S <br /> lE hw <br /> g ay 99 (14840 West Frontage Road) witha deeper well. This work is•proposed to address <br /> the SJCEHD's concern that is indicated in the attached letter. <br /> Below is a-breakdown of the proposed work: <br /> 1. New well drilling by CaIwater: $5,980 <br /> 2. Pump relocation, water& electrical line installation: $1,070 <br /> Total cost of well relocation: $7,050 <br /> Moore Petroleum had performed a similar well relocation and deepening at the subject site'in <br /> October 1997.
