The record sheet was signed, timed, and dated
by the ~ampler when transferring the sw-nples. Custody transfers were recorded for each
individual sample; for example, if samples were split and sent to more than one laboratory, a
chain-of-custody record sheet accompanied each sable. The dumber of custodians in the chain
of possession were minimized. A copy of the Sampling and Analysis Chain-of-Custody Record
was returned to EMCON with the analytical results.
Groundwater Sampling and Analysis Request Form
A Groundwater Sampling and Analysis Request Form (see Figure A-4) or purchase order, or
both, accompanied the samples delivered to each laboratory. These forms served as official
communication to the laboratory of the particular analysis/analyses required for each sample.
At a minimum, the Groundwater Sampling and Analysis Request Form included the
following information:
• Date submitted
• Specific analytical parameters
• Well number
• Sample source
SA(.W:\PJ2\252U\25201000.1 PS-94\pas:1 A-7 Rev.0.4111/95
