Correct immediately by demonstrating when the UW lamps became a UW. Submit a statement a d supporting documentation explaining how this waste was managed by October 11, 2012. Complete and submit a co y of the Return to Compliance Certification form with a statement of how each violation was correct d, including supporting documentation, by October 11, 2012. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
