EHD does require that the investigation include <br /> removal and appropriate disposal of the contents of the septic tank and that EHD staff <br /> • be present when the contents of the septic tank are removed so a visual inspection of <br /> the tank for cracks or leaks can be made prior to further use of the tank. <br /> A plan describing the proposed investigation must be submitted to the EHD by an <br /> environmental consulting firm. After completion of the investigation, a report and <br /> evaluation of the findings must be signed by an appropriately registered professional <br /> (R.G. or P.E.) and submitted to the EHD. Once EHD concurs with the consultant that <br /> the investigation/cleanup is completed you will be notified in writing and the site will be <br /> shown as closed on the list of UIC sites. <br /> Enclosed is a master file record form that is to be completed and submitted with a fee of <br /> $279 that covers the first three hours of our work on the site ($93/hour). If more than <br /> three hours is spent on oversight of the site investigation, you will be sent a bill <br /> calculated by multiplying the $93/hour fee times the number of additional hours.
