Each violation states what must be done to <br /> come into compliance. Please submit the Return to Compliance certification (attached) and any <br /> requested paperwork by 3/30/2020. If you have any questions or need assistance please let me <br /> know. <br /> As a reminder, since it is long past the 30 days to return to compliance, your facility is being charged <br /> for all time spent on bringing it back into compliance at the current hourly rate of $152.00.
