Time the" accident happened. <br />5. A complete description of the accident, includ— <br />ing: <br />a. Number of fatalities and names, if khawn; <br />b. Number of injuries and names, if known; <br />6. Car initial and number, unit number, name and <br />nuantity o' hazardous material or pollutant,. and <br />hazard class. <br />7. Whether train operations on adjacent tracks are <br />affected. <br />8. Description of the surrounding area affected by <br />the release of the hazardous material or pollu— <br />tant, including: <br />a. Nearness of populated buildings; <br />b. Nearness of other important buildings; <br />C.
