Oak Street Site Code: RO#: 1781 <br /> Stockton, CA 95205 Global ID#: T0607798060 <br /> The San Joaquin County Environmental Health Department (EHD) has reviewed Quarterly <br /> Status Summary Report— First Quarter 2011 (QMR) submitted to Geotracker. The QMR was <br /> for the reporting period between semi-annual sampling events. Notations in the report indicated <br /> no sampling, no assessment, no remediation, and no waste was generated during this reporting <br /> period. Also included in the QMR was a statement that the groundwater plume has not been <br /> adequately delineated by the existing well network. No recommendations were included to <br /> conduct an investigation to complete the required site characterization. <br /> The EHD hereby directs you to submit a work plan to continue to characterize the <br /> groundwater plume emanating from this site. The work plan is due on Geotracker by <br /> August 8, 2011. <br /> For the last two semi-annual groundwater sampling events, monitoring well MW-1S has been <br /> obstructed. Monitoring wells that are obstructed or in disrepair must be repaired and returned <br /> to usable condition immediately or destroyed and replaced under an approved work plan and <br /> permit. You are hereby directed to take immediate steps to remove the obstruction in <br /> monitoring well MWAS and return it to full usable condition. If the well cannot be restored <br /> and returned to usable condition by the next groundwater sampling event, a work plan to <br /> destroy and replace the well is due on Geotracker 30-days after the sampling event. <br /> You may also <br /> also/contact Mr.20Ta if you have any questions. <br /> /�f'�.,6?'!{Lc'.� <br /> Michael J.
