CASE SUMMARY <br /> On March 17, 1999 three underground storage tanks were removed from the property <br /> under permit and inspection by San Joaquin County Public Health Services, <br /> Environmental Health Division (PHS/EHD). Odor was detected in the tank pit, and <br /> analytical results of the collected soil samples evidenced low-level petroleum <br /> hydrocarbon contamination. The site was placed into the Local Oversight Program <br /> (LOP) on June 11, 1999. On July 23, 1999 the "Work Plan, Preliminary Site <br /> Investigation", dated July 13, 1999, was submitted to PHS/EHD. The work plan was <br /> approved and the Responsible Party was directed to submit a well permit application and <br /> schedule the work in PHS/EHD correspondence dated July 30, 1999. Since that date, two <br /> more directives have been issued by PHS/EHD to submit the well permit application and <br /> schedule the work. The Responsible Party has not complied with these directives. In <br /> correspondence dated March 29, 2000, PHS/EHD informed the Responsible Party that a <br /> meeting had been scheduled for April 17, 2000 to discuss their status of non-compliance. <br /> The Responsible Party did not appear for the meeting. <br /> II.
