Park CUF#: 01695 <br /> Stockton, CA 95202 APN: 13906035 <br /> The San Joaquin County Environmental Health Department (EHD) has reviewed Dual <br /> Phase Extraction (DPE) Pilot Study Work Plan — Addendum (Plan), dated November 15, <br /> 2010, submitted by your consultant, Advanced GeoEnvironmental Inc. (AGE). The Plan <br /> was in response to comments included in EHD's October 27, 2010 letter. <br /> As requested by the EHD, your consultant has provided modified locations for the two <br /> observation wells needed to conduct a DPE pilot test. These new locations, in addition <br /> to the original location of test well "DPE-1", meet the requirements set by the EHD. The <br /> new locations and the Plan addendum are adequate and approved. [H&S <br /> 25296.1 0(c)(3)]. <br /> The Well Permit Application form and fee have already been received at the EHD. The <br /> permit application will be processed immediately.
