(AGE). In the WP, AGE <br /> proposed into install ten ground water monitoring wells, MW-8 through MW-17, to define <br /> the vertical and lateral extent of petroleum hydrocarbon-impacted groundwater. The <br /> EHD directed that the application and fees for this work be submitted to the EHD for <br /> approval and expected the work to be implemented by January 23, 2009. The EHD has <br /> not received any permit applications or fees for this work since the October 2008 letter. <br /> Therefore, you are out of compliance with the deadline noted in the letter. <br /> Submit to the EHD the necessary well permit applications, the $89.00 permit fee and the <br /> $315.00 inspection fee for each parcel where drilling will occur; and copies of the access <br /> agreements for each off-site parcel and an encroachment permit for the boring(s) in the <br /> public right of way within 30 days of the date of this letter. Once the well permits have <br /> been approved, notify EHD of the proposed schedule for the work. The EHD requires a <br /> minimum 48-hour notice prior to initiation of any field work for the site.
