A� ) AN REVOCABLE PERMIT Ground Water Monitoring Well Permit No . : 97- 19 Date: November 7 , 1997 Location : 4405 PACIFIC AVENUE Permittee : ADVANCED GEOENVIRONMENTAL INC. 4005 N . WILSON WAY STOCKTON , CA 95205 In accordance with Ordinance 003-96, passed by the Council of the City of Stockton on January 22 , 1996 , a permit revocable at the pleasure of the Public Works Director of the City of Stockton is hereby granted to : ADVANCED GEOENVIRONMENTAL INC. to install and maintain two (2) monitoring wells at 4405 Pacific Avenue , Stockton , CA . This permit is granted under the following terms and conditions : 1 . This revocable permit is granted subject to the Standard Conditions Applicable to Revocable Permits adopted by this City Council by Resolution No . 22 , 578 on December 5 , 1960 , and on file in the Office of the City Clerk of the City of Stockton . 2. The general location of each well shall be as shown on the map attached . The precise location shall be determined in the field by the City of Stockton . 3 . The Permittee has posted security of $ 1 , 000 . 00 with the City of Stockton which will remain in effect during the life of this permit. 4 . In accordance with Section 1 ( h ) of the "Standard Conditions Applicable to Revocable Permits , " the permittee shall provide and keep in force during the term of the permit a policy of public liability insurance against liability for injuries to persons or property arising out of accidents attributable to the structure on City property, in amounts to be determined by the Risk Manager. The policy of insurance so provided shall contain a contractual liability endorsement covering the liability assumed by the Permittee by the terms of his permit and shall contain a provision that such policy may not be canceled except after thirty (30) days notice in writing to be given to the Risk Manager. The copies of these policies or certificates evidencing the same shall be filed with the Risk Manager of the City after approval of the City Attorney .
