Failed to determine!if a waste is a hazardous waste: <br /> At time of inspection: <br /> 1-55 gallon bucket with unknown contents, outside <br /> 1-ballast in a waste lamp storage box in the maintenance storage container <br /> were seen without a hazardous(waste determination. <br /> Immediately make a hazardous waste determination pursuant to Title 22 hazardous waste regulations and handle <br /> accordingly or handle as hazardous waste. Within 30 days provide written documentation describing how these waste <br /> streams were handled. <br /> 20 & 50. 1-5 gallon bucket of alkaline batteries was missing the start date and the hazardous property. <br /> The physical state was listed incorrectly. <br /> 1-55 gallon drum wast alcohol in the lab was seen without the physical state and the start <br /> accumulation date. <br /> 1-250 gallon container f used oil had the hazardous property marked as other. <br /> All hazardous waste containers hall be marked with the following information: <br /> 1. the words"Hazardous Waste" <br /> 2. name and address of generat r <br /> 3. hazardous properties <br /> 4. physical state <br /> 5. composition (contents) <br /> 6. accumulation start date <br /> Immediately label these contain rs and ensure that all containers are marked with all the required information. Since it <br /> cannot be determined how long the 5-gallon bucket of waste batteries and the 55-gallon drum of waste alcohol has been on <br /> site, and the 250 gallon container of used oil has been on site longer than 180 days (4/1/10) immediately contact a licensed <br /> hazardous waste hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD by July 11, <br /> 2011. <br /> 23. Bills of lading for the,recycling of used oil filters for the last three years were not found on site. A copy of each bill of <br /> lading must be kept on site for atlleast three years.
