The enclosed billing statement includes the 1999 Small <br /> Hazardous IN aste Pe mit fee of$100. <br /> As the Certifii d Unif ed Program Agency (CUPA) for San Joaquin County, the Environmental <br /> Health Divisi (E ) is also required to collect a state facility surcharge fee for each facility <br /> regulated and r the azardous Material Release Response Plans, Hazardous Material <br /> Management Plans, California Accidental Release Prevention Program (Cal-ARP), <br /> Hazardous aste enerator Program, Tiered Permitting Program or Underground <br /> Storage Tank (UST) Programs. If our records indicate that your facility was not previously <br /> billed for the tate fac lity surcharge of $18.50 for 1998 and/or $10.00 for 1999, these charges <br /> are included o this billing statement. <br /> If used oil is t only hazardous waste generated by your facility, you may lie eligible for a 50% <br /> reduction in60n <br /> h Small Hazardous Waste Generator Permit Fee next year.
