All work under flus permit must meet with the approval of the State <br /> Representative <br /> Failure on the Permittee's part to comply with any provision will be cause for revocation of this <br /> permit <br /> RVMIEDIATELY FOLLOWING COMPLETION OF WORK PERMITTED HEREIN, <br /> PERMITTEE SHALL FILL OUT AND MAIL NOTICE OF COMPLETION PROVIDED BY <br /> GRANTOR <br /> Permittee's attention is directed to Section 6, "CONTROL OF MATERIALS", of the State <br /> Standard Specifications Reference to Engineer in the State Standard Specifications shall include <br /> State Representative <br /> Permittee is directed to Standard Specification section 7-1 11 Preservation of Property, and <br /> Business and Professions Code, Section 8771 Permittee shall physically inspect the work site <br /> and locate survey monuments prior to work commencement Monuments shall be referenced or <br /> reset in accordance with Business and Professions Code <br /> Traffic control shall be done in conformance with Caltrans Manual on Uniform Traffic <br /> Control Devices (MUTCD) 2003. <br /> In addition to General Provision 35, "Notification of department and TMC", the Permittee <br /> shall notify the Department's Representative AND THE TRANSPORTATION <br /> MANAGEMENT CENTER (TMC) immediately prior to initiating any traffic control and <br /> immediately upon the removal of any traffic controls. The telephone number for Caltrans <br /> District 10 TMC is (209) 948-7556. <br /> A pre job conference with the Permittee, Contractor, and State Representative may be required <br /> prior to start of work Permittee shall contact the State Representative and arrange the meeting <br /> This meeting may be waived at the discretion of the State Representative <br /> Notwithstanding General Provision #4, your contractor is required to apply for and obtain an <br /> encroachment permit prior to start of work <br /> Permittee shall, prior to commencement of any work, provide the State Representative with the <br /> name and phone number of the person in responsible charge of the work to be performed <br /> under this permit <br /> This permit is not valid until Permittee has obtained permission from adjacent property owners, <br /> Imgation Districts, Cities, Counties or other interested parties to perform the proposed work <br /> Except when necessary, as determined by the State Representative, no installation, construction <br /> . equipment or personal vehicles shall operate or park within the traveled way <br /> All work shall be conducted in such a manner that the equipment will not cause any <br /> inconvenience to the highway traffic or to traffic entering the highway from any public or pnvate <br /> approach <br />
