February 14, 2003
George R. Montross, Engineer
City of Stockton
Public Works Department
425 North E1 Dorado Street
Stockton, CA 95202
Re: Revocable Permit for Canepa' s Car Wash located at 6230 Pacific Avenue, Stockton
Dear Mr. Montross:
The purpose of this letter is to document that a formal request has been made to your office, the
Department of Public Works, requesting the issuance of a Revocable Permit for the installation of a
monitor well in the City right-of-way known as Evelyn Avenue. An adjacent property located at 6230
Pacific Avenue is owned by Mr. Canepa and is in the San Joaquin County Local Oversight Program
(LOP). The owner of the LOP property, Remo Canepa, has read and understood the City' s Standard
Conditions Applicable to Revocable Permits and indicates this by providing the following attachments
that Condor is submitting on Mr. Canepa' s behalf:
• A completed copy of the Revocable Permit Application,
• A check payable to the City of Stockton in the amount of $335 .00,
• An attached Lien Bond for the amount of $500.00,
• A detailed workplan for the monitor well,
• A copy of a certificate of insurance naming the City of Stockton as an additional insured, and
• A proof of delivery indicating that the two adjacent property owners have been notified of the
proposed drilling.
I trust this letter satisfies the terms required to obtain the Revocable Permit for the monitor well. If you
have any questions or need any additional information please contact me at Condor' s Stockton office,
209.234. 0518 .
Sincerely,
Patsy Gonzalez
Environmental Specialist
P:\1707 Canepa\1707 Canepa-Pacific\Cortespondence\Revocable Permit Request.doc
