EI Dorado St., Stockton Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Complete and submit a copy of the Return to Compliance Certification to the EHD by January 3, 2013 <br /> along with a statement documenting the corrective actions that have been or will be taken for each <br /> violation and any supporting paperwork. <br /> Note: <br /> The following hazardous wastes were observed on site during the inspection: <br /> - a gallon container of potassium hydroxide <br /> - a 5 gallon bucket of spilled rodent poison <br /> - universal waste electronics <br /> - universal waste fluorescent tubes, including a cardboard box of tubes in the office bathroom, a broken <br /> tube on the office bathroom floor, and six tubes on the floor of one of the offices <br /> - laboratory waste <br /> -waste paint <br /> - a tank of used oil <br /> All hazardous waste must be removed from the facility prior to closure. Submit copies to the EHD of the <br /> disposal records for the above listed wastes and any other hazardous waste remaining on site in order <br /> to finalize the closure of this facility's hazardous waste generator permit. <br /> **Evergreen Environmental was observed on site collecting hazardous waste at the time of inspection. <br /> Provided: <br /> - CERS HMMP flyer <br /> - CERS Lead User Authorization form <br /> - Return to Compliance Certification form <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> Stacy Rivera (209) 468-3440 ��&/zaw <br /> EHD 23-02-003 Rev 07/12/11 CONTINUATION FORM <br />
