(GR) is submitting the enclosed <br /> Revocable Permit Applications for the installation of two groundwater monitoring wells in the City of <br /> Stockton's right-of-way for the above referenced site. The installation of the wells is outlined in the <br /> enclosed GR report #240072.06, Work Plan far Additional Subsurface Assessment and Remedial Action, -- <br /> dated December 11, 2001. Included with the Revocable Permit Application for the wells are a check <br /> #058085 in the amount of $310.00 (processing fee), a bond in the amount of $1,000.00 (2 wells at <br /> $500.00/well), a letter from San Joaquin County Public Health Services (SJCPHS) requesting the work, <br /> and two USPS delivery confirmation forms for work notification letters to adjacent property owners.
