Atwater <br /> Van De Pal Enterprises, Inc. <br /> Post Office Box 1107 <br /> Stockton, California 95201 <br /> Subject: Former California Fuels Cardlock Station <br /> 2402 Pacific Avenue <br /> Stockton, California 95204 <br /> Dear Mr. Atwater: <br /> The San Joaquin County Environmental Health Department (EHD) has received and reviewed <br /> Well Destruction Work Plan (work plan), dated 02 August 2012, prepared by your consulting <br /> firm, Advanced GeoEnvironmental, Inc. (AGE), on your behalf for the above-referenced site. <br /> The work plan was submitted in response to the EHD letter dated 20 July 2012 directing the <br /> submittal of a scope of work proposal that details the conditions and construction of the wells <br /> and the methods and materials to be used to destroy them. The EHD issued a 5 September <br /> 2012 letter that approved the work plan provided the work plan was revised to include the <br /> directives specified within the EHD letter. <br /> Upon further review and investigation, the EHD found that in several attempts to over-drill a <br /> multi-level chamber well (aka Continuous Multichannel Tubing or CMT) the CMT material was <br /> not completely removed, as required by Section 13.17.6 of the San Joaquin County Well <br /> Standards. The EHD is amending the destruction requirements directed in the 5 September <br /> 2012 EHD letter, due to these findings. The EHD is directing the following for the destruction of <br /> CMT well, MW-13A/B/C: <br /> • A properly licensed C-57 contractor shall install at least 100-grain explosive detonation <br /> cord through the middle channel down to the full depth of the CMT. The seven channels <br /> of the CMT shall be pressure grouted using a mixture recommended by the CMT <br /> manufacture consisting of 55 pounds (lbs) of Ultrafine Cement, 2.2 lbs of <br /> Superplasticizer, and 110 lbs of water at 25 pounds of pressure per square inch. Once <br /> all channels are grouted to surface grade, detonate the explosives and then over-drill the <br /> well using ten-inch or larger diameter augers to total depth of 150 feet below surface <br /> grade (bsg). Initially MW-13 was installed using ten-inch augers, but was over-drilled <br /> using eight-inch augers to install the CMT well. The resulting borehole shall be grouted <br /> from bottom to surface or three/five feet bsg surface with a 10.3 sack sand/ cement <br /> sealant. <br /> With this amendment the EHD approves the work plan. As stated in the 5 September 2012 <br /> letter, please submit: <br /> • An access agreement for the destruction of MW-6 and MW-10; <br /> • An encroachment permit issued by the City of Stockton to destroy wells MW-4, MW-5, <br /> MW-7 through MW-9, MW-11 and MW-12 located in the public right-of=way; <br />
