Post Office Box 1107 Stockton, California 95201 Subject: Former California Fuels Cardlock Station 2402 Pacific Avenue Stockton, California 95204 Dear Mr. Atwater: The San Joaquin County Environmental Health Department (EHD) has received and reviewed Well Destruction Work Plan (work plan), dated 02 August 2012, prepared by your consulting firm, Advanced GeoEnvironmental, Inc. (AGE), on your behalf for the above-referenced site. The work plan was submitted in response to the EHD letter dated 20 July 2012 directing the submittal of a scope of work proposal that details the conditions and construction of the wells and the methods and materials to be used to destroy them. The EHD issued a 5 September 2012 letter that approved the work plan provided the work plan was revised to include the directives specified within the EHD letter. Upon further review and investigation, the EHD found that in several attempts to over-drill a multi-level chamber well (aka Continuous Multichannel Tubing or CMT) the CMT material was not completely removed, as required by Section 13.17.6 of the San Joaquin County Well Standards. The EHD is amending the destruction requirements directed in the 5 September 2012 EHD letter, due to these findings. The EHD is directing the following for the destruction of CMT well, MW-13A/B/C: • A properly licensed C-57 contractor shall install at least 100-grain explosive detonation cord through the middle channel down to the full depth of the CMT. The seven channels of the CMT shall be pressure grouted using a mixture recommended by the CMT manufacture consisting of 55 pounds (lbs) of Ultrafine Cement, 2.2 lbs of Superplasticizer, and 110 lbs of water at 25 pounds of pressure per square inch. Once all channels are grouted to surface grade, detonate the explosives and then over-drill the well using ten-inch or larger diameter augers to total depth of 150 feet below surface grade (bsg). Initially MW-13 was installed using ten-inch augers, but was over-drilled using eight-inch augers to install the CMT well. The resulting borehole shall be grouted from bottom to surface or three/five feet bsg surface with a 10.3 sack sand/ cement sealant. With this amendment the EHD approves the work plan. As stated in the 5 September 2012 letter, please submit: • An access agreement for the destruction of MW-6 and MW-10; • An encroachment permit issued by the City of Stockton to destroy wells MW-4, MW-5, MW-7 through MW-9, MW-11 and MW-12 located in the public right-of=way;
