Please submit: <br /> • An access agreement for the destruction of MW-6 and MW-10; <br /> • An encroachment permit issued by the City of Stockton to destroy wells MW-4, MW-5, <br /> MW-7 through MW-9, MW-11 and MW-12 located in the public right-of-way; <br /> • Four Well and Boring Permit Applications to destroy the on- and off-site wells; <br /> • Master File Record form; <br /> • A permit/inspection fee of $5000.00 (28 wells x $125.00 + $375 x 4 parcels); (The <br /> EHD believes MW-4 and MW-7 are in the City right-of-way and require an <br /> encroachment permit instead of an access agreement.) <br /> In addition to the above, please provide the EHD certification in writing that all record owners <br /> of fee title to the above-referenced site have been notified of your request for closure. This is <br /> in accordance with Section 25297.15 (a) of the California Health and Safety Code.
