SECTION 7 <br /> EMERGENCY RESPONSE.IACCIDENT <br /> INVESTIGATION <br /> The phone numbers of the police and fire departments, ambulance <br /> services, local hospital, and ERM representatives are provided in the <br /> reference sheet at the back of this HSP. Directions to the hospital are also <br /> provided on the sheet. An operational cellular telephone shall be <br /> maintained on site at all times for use in the event of an emergency. <br /> Any accident/incident resulting in an OSHA recordable injury or illness, <br /> treatment at a hospital or physician's office, property damage or a near <br /> miss accident requires that an accident/incident report be completed and <br /> submitted to the ERM DIHS. The investigation will be initiated as soon as <br /> emergency conditions are under control. The purpose of this <br /> investigation is not to attribute blame but to determine the pertinent facts <br /> so that repeat or similar occurrences can be avoided. <br /> Planning <br /> Prior to facility entrance, the HSO shall plan emergency actions and <br /> discuss them with personnel conducting project work. Initial planning <br /> includes establishing the best means for evacuation from the area in case <br /> of a catastrophe. <br /> Emergency Services <br /> A tested system must exist for rapid and clear distress communications, <br /> preferably voice, from all personnel to the HSO. The HSO shall ensure <br /> that all personnel working at the facility know how to communicate with <br /> the appropriate local emergency response units as well as provide <br /> adequate and clear directions between work locations and the locations of <br /> support personnel, prior to commencing any facility investigation or <br /> operations.
