(AGE) has prepared this report for the destruction of 20 ground water monitoring wells (MW-1 through MW-10, MW-11 AB,MW-12AB,and MW-13 through MW-18),four vapor extraction wells(VW-1 through VW-4), and two ground water extraction wells (EW-1 and EW-2), at 4511 Pacific Avenue, Stockton, California(site). The site location is depicted on Figure 1; site structures and former well locations are depicted on Figure 2. Applicable San Joaquin County Environmental Health Department (SJCEHD) and City of Stockton (COS) encroachment permits are included as Appendix A. INTRODUCTION/BACKGROUND A total of 26 monitoring and remediation wells were installed at the site as part of an ongoing subsurface investigation and remediation of a former leaking underground storage tank(UST)at the site. Twenty of the site's wells were used to monitor the flow, gradient, and concentration of contaminants at the site. Four wells were used at the site to extract vapors from contaminated subsurface soils and two were used to mitigate dissolved phase petroleum hydrocarbons in ground water. The following is a brief summery of their completions and total depths. • Well MW-1 was single completed as a 2-inch monitoring well to a total depth of 78 feet bsg. • Wells MW-2 and MW-3 were single completed as 2-inch monitoring wells to a total depth of 70 feet bsg. • Wells MW-4 through MW-7 were single completed as 2-inch monitoring wells to a total depth of 50 feet bsg. 837 Shaw Road,Stockton,California 95215 Telephone (209)467-1006 Fax (209) 467-1118
