27 October 2009
AGE-NC Project No. 95-0128
A total of 8 monitoring wells(MW-5,MW-7,MW-10 and MW-14 through MW-18)were destroyed
using pressure grouting techniques. Each well that was that was pressure grouted was fitted with a
pressure tight adapter connected to a hose extending from a grout mixer. A ratio of approximately
6 gallons water for every 94-pound sack of portland cement was utilized in preparation of the grout
mixture. The grout mixture was pumped into each well casing for five minutes under approximately
30 pounds per square inch (psi)of pressure. The well boxes were then removed and the wells were
over-drilled to three feet bsg using a CME-75 HT hollow stem auger drill rig equipped with 8-inch
hollow stem augers. The top three to five feet of the well casings were then removed and the borings
were grouted to the near surface using the portland cement grout mixture.Borings for all wells were
either completely backfilled to 0.5 feet bsg with neat cement and then to surface grade with concrete
or asphalt patching. New asphalt was replaced over well vault locations and new sidewalk was
replaced over wells within sidewalks areas of the City of Stockton.
All non-hazardous waste ( i.e. soil cuttings, PVC, and well boxes) generated during destruction
activities were stored onsite in soil bins provided by Republic Services of Stockton,CA.Waste was
removed and transported to Forward Landfill of Manteca, California on 02 October 2009 by Delta
Container Corporation of Stockton, California. Remediation equipment and waste carbon was
disposed by Slaby Sales Environmental,Inc.Uniform non-hazardous waste manifests documenting
the waste disposal have been included as Appendix B.
If you have any questions or require further information, please contact our office at(209)467-1006.
Sincerely,
Advanced Ge6Environmental, Inc.
E5510RAL GFpD
R %rte
* No.-7473'
William R.
