INTRODUCTION AND SCOPE OF WORK <br /> At the request of Mr Art Wyatt, Advanced GeoEnvironmental, Inc (AGE) has prepared this <br />' Quarterly Report of monitoring activities during the second quarter of 1995 at 4511 Pacific Avenue, <br /> Stockton, Cahforma The monitoring was performed in accordance with San Joaquin County Public <br /> Health Services - Environmental Health Division (PHS-EHD) and Central Valley Regional Water <br />' Quality Control Board(CVRWQCB)guidelines for investigation at underground storage tank (UST) <br /> sites The location of the site is depicted on Figure 1 A plan of the site is depicted on Figure 2 <br />' 2.0. BACKGROUND <br /> 21 SITE ASSESSMENT <br /> The facility consists of a building and three pump islands covered by a canopy(Figure 2) In February <br /> 1991, a broken coupling in a product line was discovered and repaired During repair procedures, <br /> hydrocarbon-impacted soil was discovered beneath the product line Subsequently, the PHS-EHD <br />' required an assessment of the vertical and lateral extent of the hydrocarbon impact <br /> As a part of the assessment requirements, three soil borings, one vapor extraction well and three <br />' monitoring wells were installed at the site by Geological Audit Services Inc (GeoAudit) The vertical <br /> and lateral extent of hydrocarbon-impacted soil and ground water have been delineated to the <br /> satisfaction of the PHS-EHD <br /> 1 <br /> 2 2 UNDERGROUND STORAGE TANK REMOVAL <br /> Six USTs were removed from the site on 8 Apnl 1994 Details of the USTs removal and soil <br /> sampling were presented in a report prepared by GeoAudit In May 1994, two 12,004-gallon USTs <br /> Iand associated piping were installed at the site to replace the removed tanks <br /> I2 3 REMEDIATION <br /> A Corrective Action Plan(CAP), recommending soil vapor extraction, has been prepared for the site <br /> by Upgradient Environmental Upon approval of the CAP by the PHS-EHD, remediation activities <br />• are scheduled to be initiated <br /> I <br />
