If concrete will be broken near the tank, a secondary containment test must be <br /> performed on the affected USTs once concrete is poured in accordance with the <br /> manufacturer's guidelines. <br /> 5. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), or a <br /> Tank Tester's license issued to the individual by the State Water Board, and <br /> b. A valid, current California UST Service Technician certificate issued by the <br /> International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s) of the component(s) <br /> being tested, repaired, or serviced. <br /> 7. Qualified installers must possess or work under the direct and personal supervision of an <br /> individual physically present at the work site who possesses all of the following: <br /> a. A valid, current contractor's license issued by the Contractors State License Board, <br /> and <br /> b. A valid, current certificate of training from the manufacturer(s) of the component(s) <br /> being installed, and <br /> c. A valid, current UST Installation/Retrofitting certificate issued by the International <br /> Code Council (ICC). <br /> 8. Update and submit a copy of the "UST Monitoring Plan" for EHD approval. An approved <br /> copy will be returned for on site records. <br /> 9. A "UST Certification of Installation/Modification"form shall be submitted by the owner or <br /> the owner's agent (CCR Section 2635(d)) before this upgrade is considered complete by <br /> EHD. <br /> 2 <br />
