California Blvd., Suite 650 <br /> Walnut Creek CA 94956 <br /> RE: UNION OIL S/S#5421 IN REPLY REFER TO: SITE CODE: 1226-123 <br /> 7303 PACIFIC AVE. <br /> STOCKTON 95207 <br /> The San Joaquin County Public Health Services, Environmental Health Division <br /> has recently been included in the State Water Resources Control Board (SWRCB) <br /> UST Pilot Program for oversight of assessment and clean up activities at <br /> contaminated underground tank sites. <br /> A proposal for initial assessment and/or remediation for the above referenced <br /> site has been submitted to this office. <br /> All proposals and reports submitted must be accompanied by a Letter of <br /> Acknowledgement from the responsible party(ies) which states, at a minimum, <br /> the following: <br /> I(We) declare, under penalty of perjury, that the information <br /> and/or recommendations contained in the attached proposal or <br /> report is/are true and correct, and that all work and reports <br /> which required geologic or engineering evaluations and/or <br /> judgments have been performed under the direction of an <br /> appropriately registered or certified professional. <br /> The attached proposal or report utilizes the "Regional Board Staff <br /> Recommendations For Initial Evaluation and Investigation of <br /> Underground Tanks" and "Appendix A" reporting format. The LUFT <br /> Manual has also been utilized as a guidance document. <br /> Furthermore, I(We) agree to submit, on the date noted, the <br /> following on the site(s) in question: <br /> i. . . . . . . . .Soil Contamination Work Plan" and "Soil <br /> Remediation Plan" by (datel to San Joaquin <br /> County PHS/EHD. <br /> and/or <br /> ii. . . . . . ."Preliminary Report" by (datel to the <br /> A Division of San Joaquin County Heatrh Care Services <br />
