The <br /> augers need to be sized sufficiently to reach out and remove all well casings, <br /> screens, filter packs, and grout sealant installed during construction. <br /> • This site has one well located off-site in the City of Stockton right-of-way. Please <br /> submit a copy of a valid encroachment permit when submitting the EHD Well and <br /> Boring Permit Application form for these wells. <br /> After the EHD has approved the well destruction scope of work proposal and the public <br /> participation comments have been properly addressed, you may submit an EHD Well and <br /> Boring Permit Application form along with a Master File Record form, Encroachment Permit <br /> issued by the City of Stockton, and appropriate fees for each parcel. The permit fee for the <br /> destruction of each well is $125, along with a $375 Inspection Activity fee per parcel. A <br /> review of the site records indicate that six wells currently are installed. (To determine the <br /> fees for nested wells, each nested well is considered as one well for permitting and fee <br /> purposes.) A fee of$1,500.00 (6 x $125 + $375 x 2) will be required for the well destruction <br /> permits. <br /> The EHD will issue a closure letter once the sixty-day public participation notification <br /> requirements and opportunity for public participation in the case closure decision process <br /> have been completed in accordance with the LTCP, all wells have been properly destroyed <br /> and waste manifests have been submitted to this agency. <br /> Please contact Vicki McCartney at (209) 468-9852 or by email at vmccartney(a).sicehd.com if <br /> you have any questions. <br /> Sincerely, <br /> Victoria L.
