r <br /> July 14,2014 <br /> Page 3 of 4 <br /> Reference: Well Destruction Report <br /> 7-Eleven Store #17334 <br /> 4501 North Pershing Avenue <br /> Stockton, California <br /> Waste Disposal <br /> Soil and debris generated during well destruction activities was temporarily stored onsite in <br /> properly labeled 55-gallon drums.The soil drums were picked up by Belshire Environmental <br /> Services, Inc. on May 28, 2014 and transported to Soil Safe, in Adelanto, California on June 9, 2014. <br /> Decontamination water was temporarily stored in a 55-gallon drum; this drum was also picked up <br /> by Belshire on May 28, 2014, and transported to Demenno Kerdoon in Compton, California on <br /> June 9, 2014. Copies of waste manifests are included in Attachment E. <br /> Summary <br /> On May 15 through 20, 2014, Stantec supervised National EWP during the destruction of four <br /> groundwater monitoring wells and two remediation wells. Debris generated during well <br /> destruction activities was properly disposed of offsite.The destruction of the wells at this location <br /> was conducted pursuant to DWR:Bulletin No. 74-81 and Supplement No. 74-90, and under the <br /> direction of a registered California Professional Geologist., <br /> Based on compliance with the California Regional Water Quality Control Board (CRWQCB) and <br /> SJCEHD closure requirements, and on behalf of 7-Eleven-Inc., Stantec requests final closure of the <br /> site. <br /> Limitations <br /> This report was prepared in accordance with the scope of work outlined in Stantec's contract and <br /> with generally accepted professional engineering and environmental consulting practices existing <br /> at the time this report was prepared and applicable to the location of the site. It was prepared for <br /> the exclusive use of 7-Eleven, Inc., for the express purpose stated above. Any re-use of this report <br /> for a different purpose or by others not identified above shall be at the user's sole risk without <br /> liability to Stantec.To the extent that this report is based on information provided to Stantec by <br /> third parties, Stantec may have made efforts to verify this third party information, but Stantec <br /> cannot guarantee the completeness or accuracy of this information. <br /> I <br /> Design it <br /> g with community in mind <br />
