Underground Storage Tank Removal Report and Work Plan for Monitoring Well Installation 1 <br />. Southland Store#17334,4501 North Pershing Avenue,Stockton,California --_ October 27 1998 <br /> 1 0 INTRODUCTION <br /> Fluor Daniel GTI, Inc has prepared this report summarizing underground storage tank(UST) <br /> replacement activities conducted at the Southland Store No 17334, located at 4501 North Pershing <br /> Avenue, Stockton, California (Figures 1 and 2) The scope of work Included <br /> 1) Collection and analysis of sod samples from the former and new UST excavations at the <br /> site (Table 1) <br /> 2) Collection and analysis of soil samples collected from stockpiled soil generated during the <br /> removal of the USTs and Installation of the new USTs <br /> 3) Report preparation documenting the UST replacement at the site <br /> Soil sampling was conducted in accordance with the Tri-Regional Guidelines and under the direction of <br /> Mr Michael Kith and Mr Harlin Knoll of the San Joaquin County Public Health Services (SJCPHS) <br /> The UST removal, soil excavation, and Installation of new upgraded USTs was coordinated by the <br /> Maness Corporation Field notes are included in Appendix A <br /> In addition, this report presents a work plan for installation of a double-nested groundwater monitoring <br /> well to be installed within the former tank pit area at the subject site as requested by the SJCPHS The <br /> well will be used to replace wells MW-1 and MW-1A which were destroyed prior to removal of USTs at <br /> the site to avoid possible damage during the UST excavation The scope of work Includes drilling and <br /> sampling of one soil boring, Installation of two monitoring well casings screened at separate depth <br /> intervals, collection of groundwater samples from each boring, and preparation of an assessment <br /> report summarizing the methods and results of the work preformed <br /> A well search will also be conducted to locate all water supply wells within a 2,000-foot radius of the <br /> site <br /> 2 0 PERMITTING/SITE-SPECIFIC HEALTH AND SAFETY PLAN <br /> Fluor Daniel GTI conducted a technical review of all relevant information prior to proceeding with sod <br /> sampling and UST removal Applications for permits to remove the USTs were filed by the <br /> construction contractor and approved by the SJCPHS <br /> Following a complete review of site conditions, Fluor Daniel GTI revised the existing site-specific Health <br /> and Safety Plan as required by the Occupational Safety and Health Administration (OSHA) Standard <br /> "Hazardous Waste Operations and Emergency Response" guidelines (29 CFR 1910 120) The <br /> document was reviewed and signed by all Fluor Daniel GTI personnel prior to commencement of work <br />. at the site <br /> 17334ust rpt <br /> FLUOR DANIEL GTI I`5_ <br /> i�, <br />
