The following describes the <br /> violation items you noted during your inspection as well as corrective actions taken: <br /> Item #107 - An accurate UST Monitoring Site Plan was not submitted. Accurate <br /> location of annular sensor is not identified. <br /> On 02/20/2018, BP vendor, Belshire Environmental Services, Inc., updated the UST <br /> Monitoring Site Plan to show correct location of all sensors and submitted the updated <br /> Plan to CERS. A screenshot of the CERS submittal as well as a copy of the updated Plan <br /> is enclosed herein for your review and records. <br /> Item #206 — The S11 Vacuum sensor was replaced on 01/11/2018 and a permit was <br /> not obtained from the EHD. <br /> On 02/06/2018, BP testing and maintenance vendor, Gettler-Ryan, Inc., submitted a <br /> permit application and associated fees for the replacement of the Veeder-Root Vacuum <br /> Sensor for 91 product line secondary containment. This was received by your <br /> department and processed under Service Request #SR0078804. As of submittal of this <br /> letter, BP/Gettler-Ryan is awaiting the issuance of permit. A copy of the permit <br /> application is enclosed herein for your records. <br />
