Islamic Center Of Stockton -2- <br /> • The Fund's technical staff will need to review the circumstance, which led to the <br /> release/spill/emergency response that occurred at this site and has requested that <br /> you provide additional information for their review. Provide complete copies of all <br /> technical reports, (including the latest) and any prior site assessment reports that <br /> may have been done for this site. Claimant must explain in'detail the circumstances <br /> surrounding the cause of the release/spill and how the water got into the fill tube that <br /> led to UST (had the UST been previously decommissioned?). Provide any <br /> pictures/drawings showing the locations of the water hydrant and UST (how did the <br /> water hydrant come to release water?); and provide map showing the location of the <br /> UST and water hydrant. <br /> • Provide documentation to show that the subject UST had been previously <br /> decommissioned' prior to January 1, 1990. <br /> OR PROVIDE THE FOLLOWING: <br /> • A copy of the removal permit for all underground storage tanks listed in claim <br /> application. <br /> • A copy of the permit to own or operate the UST from the local implementing agency <br /> dated between January 1, 1984 and January 1, 1990 (pursuant to Chapter 6.7 of the <br /> Health and Safety Code). <br /> If you were not subject to the permit requirement, submit documentation to confirm <br /> this claim. Situations where the permit was not required by January 1, 1990, can <br /> include: a) you removed all USTs prior to January 1, 1990; and not replaced; b) you <br /> decommissioned all USTs pursuant to the direction of the regulatory agency prior to <br /> January 1, 1984; c) you sold the property and tanks by January 1, 1990. <br /> If you were subject to the permit requirement but failed to comply by January 1, <br /> 1990, you can request the State Board to waive the requirement as a condition for <br /> eligibility. To request a waiver, complete the enclosed "Permit Waiver Request" <br /> form and return with any additional information requested below. Please note that <br /> permit waivers authorized in section 25299.57 are only available.for permits that <br /> were required by January 1, 1990, and may not be used to excuse permit non- <br /> compliance that lasted beyond January 1, 1990. <br /> • A copy of the latest permit to own or operate the UST from the local implementing <br /> agency dated prior to the removal permit. <br /> A"decommissioned"tank is one that cannot have inputs or withdrawals, for one or more of the following reasons: <br /> 1)it has been filled with an inert solid;2)its fill pipes have been sealed, or 3)its piping has been removed.
