Dozier(claimant's attorney)on October 17,2005. Ms. Knieriem discussed with Mr.Dozier <br /> additional information needed for further review of your claim application to the Fund. As of this date <br /> the Fund has received no response to the October 17, 2005 telephone conversation. After reviewing your <br /> claim application to the Cleanup Fund,we find that the following additional information is still needed to <br /> determine your eligibility for placement on the Priority List: <br /> • Claimant must provide the following for the purchase of the site: <br /> 1. Purchase Offer <br /> 2. Purchase Agreement(final) <br /> 3. Appraisal Report <br /> 4. Escrow Instructions <br /> • On page one of your claim application, remove City of Stockton as Co-Payee(the Court Order <br /> documents submitted for Third Party Compensation will be reviewed by our Settlements staff after <br /> we have received additional information requested in this letter). <br /> • Claimant is requesting priority class "B" for a Non-Profit Organization. Provide latest federal tax <br /> documents(form 990 or 995)to show claimant's annual revenue does not exceed $7 million in annual <br /> gross revenues. <br /> • Claimant must provide an "original signed"Verification and Signature Page including signatory's title <br /> as related to the claimant(see enclosed Verification and Signature page); copies are not accepted. <br /> • The Fund's technical staff will need to review the circumstance,which led to the <br /> release/spill/emergency response that occurred at this site and has requested that you provide <br /> additional information for their review. Provide complete copies of all technical reports, (including <br /> the latest)and any prior site assessment reports that may have been done for this site. Claimant must <br /> explain in detail the circumstances surrounding the cause of the release/spill and how the water got <br /> into the fill tube that led to UST(had the UST been previously decommissioned?).
