Page 1, paragraph 2: Revise the second sentence to read: "NMs with measurable free-phase petroleum product shall be corrected.for project thickness to obtain an approximate depth to ground water." Page 1, note 1: Revise the first and second sentence to read: "From wells which have not been monitored for dissolved (filtered) or total lead. If .dissolved or total lead is detected, the Discharger shall resample the well for dissolved lead during the next quarterly sampling event." Page 1, paragraph 3: Insert after the first sentence: "The first calendar quarter shall begin during the month of _ ". (Insert the month that this program is finalized in the blank space). Page 2, paragraph.1: Revise the first sentence to read: "Prior to startup of a ground water treatment system, if any, the Discharger...." •i Page 2, paragraph 2: Revise the paragraph to read: "An annual report evaluating the effectiveness and . progress of investigation and remediation.(if any) shall be.submitted to the Board by the 30th day of the month following the fourth consecutive quarterly sampling event and may be submitted for.the quarterly monitoring report. The report shall contain the following information, if applicable: tabular summaries of.the monitoring data obtained during the year; a graphical.summary of the monitoring data for the key indicator compound(ie., TPH/D or benzene) obtained during the previous.year if it is consistently detected, groundwater contour maps and contaminant concentration maps'(if concentration contours can be developed); a discussion of trends in the concentrations of the pollutants in groundwater monitoring .wells; whether the contaminant plume is being captured by an extraction system or is continuing to spread; plans as needed for improving the groundwater extraction .or treatment system; and the anticipated date for completion of cleanup 'activities.
