i <br /> Advanced GeoEnvironmental, Inc. <br /> November 17, 1999 <br /> On November 10, 1999, PHS/EHD was additionally informed that MW-7, the <br /> northernmost MW in the work plan would not be completed until the analytical <br /> results of the soil samples from the other monitoring wells were reviewed and it was <br /> determined that MW-7 was still necessary. PHS/EHD was not consulted before these <br /> decisions were made. <br /> R <br /> At the meeting on November 16, 1999, PHS/EHD was informed that AGE planned to <br /> install another MW not proposed in the work plan next to MW-6, which was installed <br /> 4 in the continuous cored bore-hole. As part of the work plan approval, PHS/EHD <br /> provide the condition that the proposed boring (continuous core boring) near B5 <br /> should advance to at least 30 feet below surface grade with the contingency to bore <br /> deeper if necessary to provide vertical definition and that if it was necessary a MW be <br /> installed in separate boring. Installation of a second separate MW next to MW-6 <br /> deviates from this condition. <br /> Communication about site corrective action decisions and the scheduling ofwork <br /> between PHS/EHD and AGE must improve. PHS/EHD approval should be obtained <br /> before any modifications to the work plan are made.
