RO#: 550 <br /> Lodi, CA., 95242 CUF#: 12992 <br /> San Joaquin County Environmental Health Department (SJCEHD) has reviewed both <br /> Work Plan Addendum — December 2004 and Revision to Work Plan Addendum, dated <br /> 31 December 2004 as submitted by your consultant, Advanced GeoEnvironmental Inc <br /> (AGE) and has the following comments. <br /> The original work plan submitted to SJCEHD on January 18, 2005 was in response to <br /> requirements for continued investigation to define the lateral and vertical extents of the <br /> ground water petroleum plume. This work plan included installation of three (3) multi- <br /> level (CMT) monitoring wells onsite. This type of well is a single continuous cored <br /> polyethylene casing, divided into seven longitudinal segments. SJCEHD required that <br /> an effective plan to remove/destroy this type of casing be approved by SJCEHD prior to <br /> approving their installation. The revision to the original work plan, dated March 16, <br /> 2005, included sonic drill techniques for the eventual well destruction and <br /> recommended using bentonite grout instead of a cement grout with the installation of <br /> these wells at this site. <br /> SJCEHD hereby approves the original work plan and revision as adequate and <br /> necessary elements for the required investigation.
