The Underground Storage Tank <br /> Cleanup Fund (USTCF) has requested additional information to process the claim submitted by Mr. <br /> Zarakani. Specifically,the USTCF has requested the following information: <br /> I. Copy of the letter from the PHS-EHD naming Mr. Zarakani (D.I.K. Enterprises, Inc.) as a <br /> responsible party: <br /> 2. A copy of the permit to own or operate the UST',dated between January 1, 1984 and January 1, 1990 <br /> (from Bokides); <br /> 3.
