To: Tim Cuellar <br /> Subject: 14749 Thornton Rd., Lodi (3 B's) Permit <br /> Hi Tim, <br /> I have received the following for the destruction of nine CMT wells(MW-7 through MW-15): permit application, LCD, <br /> copy of C57 license, copy of check and MFR.The permit application has a comment"See work plan dated 09 May 2014." <br /> However, Well Destruction Work Plan Addendum, dated 09 May 2014,was not approved as stated in EHD email dated <br /> July 2,2014(the email is posted on Geotracker). In the same email,sonic drilling was the approved by EHD as the <br /> method to overdrill the CMT wells. Updated supporting documentation was not included with the permit application <br /> package, such as a site plan and table with well construction details/planned method of destruction as requested in EHD <br /> email October 13, 2014. <br /> In addition, Well Destruction Work Plan Addendum stated, "...15.25-inch hollow stem augers will initially be utilized <br /> (CME-95 hollow stem <br /> drill rig)at wells MW-11 and MW-12 to over-drill to each grout seal depth;wells MW-13, MW-14 and MW-15 already <br /> have 15-inch stainless steel conductor casing installed to total depth of each grout seal."This information does not agree <br /> with information presented in Multi Level Monitoring Well Installation Report- Third Quarter 2005, which states, "for <br /> wells MW-13 through MW-15,an 8-inch diameter steel conductor casing was installed at each of the above referenced <br /> well locations; each conductor casing was place inside a twelve-inch diameter borehole to eleven feet bsg..."Also,well <br /> design documents in the same report show a 12-inch diameter in the upper 11 feet and an 8-inch diameter steel <br /> conductor casing for those three wells. <br /> All material within the original boreholes should be removed, including the steel conductor casings installed in MW-13, <br /> MW-14,and MW-15. Prior to issuance of the well destruction permit, please clarify via email the type of drilling rig to be <br /> utilized for the removal of the nine CMT wells and all auger sizes to be used in the process(e.g.size of auger to be used <br /> on MW-13,-14,and-15 to remove the upper 11 feet).An updated site plan and well summary table would be helpful as <br /> well for inspection purposes,as those included in the Well Destruction Work Plan Addendum includes information which <br /> is no longer applicable.
