Mr. Michael lnfurna 3 April 13, 2004 <br /> Monitoring Well Destruction <br /> Monitoring wells MW-1 through MW-3 are proposed for destruction by over-drilling to remove <br /> the casing, grout,bentonite seal, and sand pack used to construct each well. Shaw personnel will <br /> review well construction logs and measured well depths prior to destruction to confirm the <br /> proper drill-out depth. The wells will be over-drilled using 8-inch hollow stem augers to <br /> approximately 23 feet bsg. Concrete slurry will then be tremmied into the boreholes to <br /> approximately 6 inches below surface grade under the supervision of a representative of the <br /> SJCEHD. The boreholes will then be capped with concrete at the surface. Copies of SJCEHD <br /> Well Destruction Completion Notices and Department of Water Resources (DWR) 188 Reports <br /> will be forwarded to appropriate regulatory agencies. <br /> i <br /> Waste Disposal <br /> Soil cuttings generated during well destruction activities will be temporarily stored on-site in <br /> labeled, DOT-approved 55-gallon drums pending transport to an approved disposal facility. The <br /> soil will be disposed of using the profile data from the September 2003 well installation <br /> activities.
