f • <br /> CAMBRIA <br /> Work Tasks <br /> Permit An appropriate permit for well destruction will be obtained from EHD <br /> Site Safety Plan Cambria will prepare a Site Safety Plan for field work <br /> Utility Clearance: Cambria will mark proposed work location and clear it through Underground <br /> Service Alert prior to commencing work <br /> Well Destruction The well to be destroyed is a six-inch diameter steel cased water supply well <br /> The well was properly abandoned in the past by welding a steel plate over the top of the well <br /> casing The steel plate has already been removed and replaced with a PVC slip cap Gregg <br /> Drilling and Testing, Inc (Gregg), of Martinez, California will be the drilling contractor <br /> performing the well destruction activities A Cambria geologist will supervise the destruction <br /> activities <br /> The steel casing will be perforated, by Gregg, utilizing a casing ripper tool Cement grout will be <br /> placed in the well from the bottom up The perforations in the casing will allow the cement grout <br /> to penetrate the soils surrounding the well casing The well casing will be cut off three feet <br /> below grade The well casing will be topped off with cement and the cement will be allowed to <br /> flow over the top of the casing creating a mushroom cement cap at the well head The remaining <br /> hole will be backfilled with native soil up to the ground surface <br /> Any water generated from the well during well destruction activities will be contained and stored <br /> onsite in drums or other suitable container pending proper disposal <br /> Report Preparation Following completion of well destruction activities, Cambria will prepare a <br /> report which documents the well destruction and submit it to EHD <br /> The scope of work described in this work plan will be performed under the supervision of a <br /> registered geologist or engineer <br /> Schedule <br /> Cambria is prepared to begin work upon written approval of this work plan by EHD and receipt <br /> of an appropriate drilling permit We have scheduled this work for February 12 and 13, 2003 <br /> 0813 2 <br />
