The permit has been approved with the following conditions. <br /> Note: EHD verified functionality of this sensor on September 17, 2008. <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution <br /> Control District for any applicable permits and requirements. <br /> 2. Contact Michelle Henry (209) 953-7699 to arrange the following inspection <br /> appointment (a 48-hour notice is required for inspection appointments): <br /> a. EHD must witness the functionality of the monitoring system to ensure <br /> that monitoring has been restored to meet the requirements of California <br /> Code of Regulations Title 23 and the Health & Safety Code. <br /> 3. Any changes in information and scope of work must be submitted to EHD for <br /> approval prior to commencing work. All new addenda supersede the previous <br /> ones. <br /> 4. All Components installed must be certified by an independent testing organization <br /> and bear the appropriate mark as required by the independent testing organization. <br /> 5. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all <br /> of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by <br /> the Contractors State License Board (may be issued to the installation <br /> company), and <br /> b. A valid, current California UST Service Technician certificate issued by <br /> the International Code Council (ICC), and <br />
