Louise Ave. <br /> Your permit application to modify the fill and vapor risers on the three underground storage <br /> tanks at the above referenced location has been approved with the following conditions: <br /> 1. Contact the Fire Department and San Joaquin Valley Air Pollution Control District for <br /> applicable permits. <br /> 2. At least one person who holds the International Code Council UST Installer/Retrofitter <br /> certification must be onsite when work is performed. <br /> 3. Each person installing or testing any component must hold a certificate of training from <br /> the manufacturer of that component. <br /> 4. Contact EHD to schedule the following appointments: <br /> a. The spill buckets must be tested to ensure they hold a minimum of 5-gallons and <br /> pass a 60 minute lake test or equivalent. Allow 48-hour advance notice for <br /> scheduling inspections with this office. <br /> 5. If concrete is broken above the tank pit, a tank integrity test must be performed by a <br /> certified tank tester and results received by this office within 7 days of completion of <br /> work. <br /> 6. Update and submit a copy of the Monitoring Plan no later than 30 days after work is <br /> completed. <br /> 7.
