Regulatory oversight will be provided by PHS/EHD's Corrective Action <br /> Program (CAP). Please complete the enclosed form, "Site Mitigation Acknowledgemenr/Request for <br /> Services" and submit it with the minimum 5 hour$390 fee ($78 per hour). You will be billed quarterly <br /> at a rate of$78 per hour for oversight time in excess of the initial five hours. <br /> Please submit a work plan to continue the investigation and remediation of documented contamination <br /> by February 15, 1993.
