A copy of"Health and Safety Plan". <br /> 2. Completed page 3 of the Closure Permit application. Name of the local Fire District and the fire <br /> permit number, name of the third-party laboratory and the third-party sampling firm are lacking. <br /> 3. Completed page 4 of the Closure Permit application. Information on decontamination <br /> procedures needed to be provided in section 7a-d. <br /> 4. Correct hazardous waste hauler registration number. On the Closure Permit application, <br /> sections 6, 7e, 8b of page 4 and section 11 c of page 5 lists incorrect Hauler Registration <br /> number for Belshire Enviromental Services Inc. <br /> 5. Tank/ piping disposal site information on page 5 of the Closure Permit application section 8c <br /> has an incorrect address for Clean Harbors Environmental. The EPA ID number listed does <br /> not belong to Clean Harbors. <br /> 6. Completed "Authorization to Release" form. The laboratory name is missing. <br /> 7. Completed "Backfill Excavation Certification".
