' 0 <br /> A chain-of-custody record will be filled out for each shipment of samples to be sent to the <br /> laboratory for analysis Information contained on the triplicate carbonless form will include the <br /> tfollowing- <br /> 0 Date and time the sample was taken. For QA/QC samples, such as replicates and <br />' splits, the actual sample time shall be recorded, not the time of the primary <br /> sample <br /> ' o Sample number and the number of sample bottles <br /> o Analyses required <br /> ' o Remarks including preservatives added and any special conditions or specific <br /> quality control measures. <br /> ' o Container number in which sample had been packaged <br /> o Labeling of travel blanks. <br /> ' Blank spaces on the chain-of-custody record will be crossed out between the last sample number <br /> listed and the signatures at the bottom of the sheet <br /> After carefully packaging the samples into numbered containers (e.g , ice chests) for transfer <br /> to the laboratory, the field sampler will sign the chain-of-custody record and record the time and <br /> the date. The original imprint of the chain-of-custody record will accompany the sample <br /> ' containers. Following review by the project manager or task leader, a duplicate copy will be <br /> placed in the project file <br /> 6.0 EQUIPMENT CLEANING <br /> ' Sample bottles and bottle caps will be cleaned by the subcontracted laboratory using standard <br /> EPA approved protocols Sample bottles and bottle caps will be protected from contact with <br /> ' solvents, dust, or other contamination between time of receipt by B&R and time of actual usage <br /> at the sampling site. <br /> t Sampling equipment that will be reused shall be cleaned/decontaminated after sampling of each <br /> well.
