Please submit a Monitoring Well Destruction Work Plan, <br /> yen-nit application and $60.00 permit fee to PHS/EHD by May 18 2000. <br /> Once the MWs have been destroyed, a"No Further Action" letter will be prepared by <br /> PHS/EHD.
